Puppy Application

Thank you for your interest in our Poodles & Cockapoos. I require that my puppy questionnaire be completed in order to be considered to be put on the waiting lists for a puppy. Once we receive your Questionnaire form, I will contact you within 48 hours to discuss it and provide details about our current puppies and upcoming litters. No puppy will be held without a filling out my puppy questionnaire and sending a deposit first. Pet price on all puppies is $1,500
